Ear Examination ENT is often a challenging examination, crossing over with the cranial nerve examination of the vestibular cochlear exam as well at other neurological assessments of balance
Here we will review the ear examination, looking both at the use of the otoscope, but also the Dix-Hallpike Manoeuvre, along with HINTS assessment. the Webers and Rinne's test is also included to determine types of hearing loss.
Often these ear examination techniques are performed separately, depending on the patients presenting complaint

hello
my name is dr gill i've been asked to do
an examination of your ears today
uh before we go any further could you
confirm to me your name and date of
birth yeah it's i'll be talk to the 7th
of december 96.
thank you so in terms of having a look
at your ears today that'll involve me
literally having a look it'll involve me
putting a speculum into your ears and
seeing what the tympanic membrane your
eardrum looks like
also putting my hands either side of
your head and moving you around on the
couch
and then also seeing how your internal
ears are working would that be right
yeah
so to start off with do you have any
problems with your hearing at the minute
do you have any added sounds in your
there's hearing pain in your ears okay
so what i'm going to do
i'm going to ask you to close your eyes
and i'm going to put my hands either
side
of your um head and i want you to tell
me if you can hear something being
whispered
okay so if you close your eyes please
42 42 okay we're going to do the same on
the opposite side
99 99 excellent
okay so we're going to just uh get this
speculum
and i'm just going to have a quick look
at your actual ears
so everything appears normal i can't see
any odd positions if you can move your
hair backwards please
okay and there's no bruising on that
okay so is there any pain or tenderness
at the front there
no okay and if i pull back like that
nope
okay so i'm just going to put the
speculum in here
we're going to go in and have a look
and have a good look around
okay any discomfort at the back there no
okay do the same again
can't see anything underwater if you can
move your hair back please
all right thank you can't see any
obvious bruising there's no redness
i'm going to touch your hair if you
don't mind so i'm just going to press
over the front of the air
any pain or tendons there no and if i
pull back
like so any problems no super
i'm just going to put the speculum in
i'm just going to have a look around
and i can't see anything that's all good
so what i'm going to do now if you'd be
if you're happy i'm going to step behind
you i'm going to put my hands on your
neck
and have a look for any lymph nodes is
that okay
so i'm just going to step behind we're
going to have a little look at the lumps
and bumps could you move your hair
backwards please
okay so we're going to start off in
front of the ears tell me if there's any
pain or discomfort
and then behind the ears if i tap there
any problems
if i tap there any problems no okay so
just going to feel around here
so i'm just going to check around here
and just raise your shoulders up for me
that's fine and back down
need to go under around the chin
and jaw and then up at the back of the
head
that's all good so what we now need to
do
uh we're going to have a look how your
internal ear is working
so again i'm going to step behind you
i'm going to get you to lie back but i'm
going to hold your head in a position as
we do that
i'm going to lie you down and i need to
have a look at your eyes as you do that
thank you so before we start that do you
have any problems with your neck
no super if you could swing your legs up
please
so i'm going to put my hands on your
head and turn you toward me
and then we're going to lean you back
head all the way back
that's ground we'll sit up and we'll
come over this side
and then all lie down
that's fine okay can you sit up if you
turn your legs around please
so the next part of our examination uh i
need to again
see how uh your balance is so i'm going
to put my hands either
side of your head and turn your head
okay
so i just want you to keep focused on
the bridge of my nose i'm going to put
my hands either side of your head
in order to stay looking at that bridge
of my nose
okay
okay that's fine there's nothing
abnormal there
our final test i'm going to take the
tuning fork and just strike it and i'll
put this in the middle of your forehead
when we do that you'll hear a noise
please tell me which ear you hear it in
is it
left the right or is the sound equal in
the middle
and tell me which side you can hear the
noise in is it left right or middle
equal in the middle excellent so we're
going to do the same again
but i'm now going to check the ears by
the side so if you can move your hair
back for me
i'm going to put at the back of the head
okay tell me when you can't hear it
anymore
stop okay can you hear it now yes okay
i'll do the same again and you should be
able to hear the noise
yeah tell me when you can't hear the
noise
stops okay can you hear it now yes
excellent well that completes our
examination of your ears i didn't find
any abnormal findings
do you have any questions to myself
thank you very much
