Ever since I spoke with Dr. Now, I've been doing the diet.
It's been a tough adjustment, but I'm getting used to it.
I've also been doing physical therapy a few times a week for the past month.
It's been hard because of the pain.
Are you ready for Jason?
All right. Thank you.
You're welcome. If you need anything else, let me know, okay?
All right.
And I tell Jason, the PT guy, that I have to stop sometimes
when the pain gets too much for me to be able to move a lot.
What do you think you guys are going to work on today?
We're working on trying to get my legs moving as best as they can.
Come in.
Hey.
Hey, man. How are you doing?
Good. How's it going?
Anything out of the ordinary?
Same stuff?
No, I'm good. I guess my knee hurts a little bit.
Okay.
I'm okay.
Okay.
I'm fine.
Right on. Okay, good.
Trying to move as much as you can.
Okay. Bring this knee up.
All right. Bring it up again.
Hold. Hold for me.
Come down slow and easy.
Ready?
I'm going to straighten that knee.
And then push down on me like you're pushing on the gas.
All right, big man.
All right.
Push on me.
Push, push, push, push, push, push, push, push.
Good. Come up with me.
Good. Then we'll come back down.
Good, man.
I'm hoping that the more I do it, the less it'll hurt
because I want to get out of this bed.
But I feel like I'm making some progress with my legs,
a little bit at least.
One of the big challenges is going to be the range of motion
that is in his ankles.
That is going to take a significant amount of work
for him to be able to have a unsupported,
supported seated position
and eventually be able to stand up.
It should feel like work.
It is?
It means you're doing it right.
Good. Yeah.
It means you're doing it right.
Ready? Come on.
Go. Strong.
Come on, bud.
Good.
I know the doctor was concerned to be able to wave off
the possibility of blood clots.
He wants him to have that much more mobility.
That's definitely something that needs to be addressed.
All right, increase your pace just a touch.
There you go, bud.
Good.
Keep your breath moving.
Don't hold your breath.
Good. Keep going.
Let's go 10 more seconds.
Jason has given me a great workout.
He got me the spark to pull up
and he has me doing all sorts of exercises
to get my heart rate up and to build up my stamina.
And that all is going great.
All right, man.
Good. Keep working hard, okay?
All right, you got it.
We'll talk to you later.
Today I'm going to see Abby
and see what's going on with him.
I had a virtual call with him a couple months ago
and gave him the diet.
Then I started working on his eating habit
and I arranged PT to start working with him
to see how much progress he could make
with his weight loss and improving his mobility.
And now after two months,
we will see how far he has come with that.
If he works hard,
we should have some hopeful situation
with him be able to walk and change his life.
I'm having physical therapy.
Come join me.
And I'm going to assess the situation
with physical therapy
and see what option we got for Abby.
Hopefully, we will see some level
of positive improvement with him today.
Hello, how are you?
Good, how are you?
Hi, Abby.
Hey, doctor.
How are you?
Good to meet you in person.
All right, you doing okay?
I'm doing much better now.
I think I'm doing pretty good.
I'm already tired of salads,
but I'm looking with my wife
to see if we can find something else
that we could eat that's healthy.
What do you eat every day?
A lot of salads,
at least at lunch and not dinner, but...
Okay, so what are you drinking?
Water mostly, yeah.
Okay.
He likes to drink water a lot.
All right, so no sugar and no juice?
No.
All right, how are we doing
with your exercises and physical therapy?
I've been doing my exercises.
This is why I have this here
and this to...
Breathing exercises?
Breathing exercises,
but it helps the core.
That's what the physical therapist told me,
so I've been doing those
since he got me those things,
so I've been doing those exercises.
All right, let me check a couple things
and see how things are.
Take these back.
Take another one.
All right, and do you have any stomach pain
or anything like that?
No, no.
Do you have a feeling down here when I touch?
Yes.
You feeling okay with that?
Yeah.
You have a lot of swelling in your feet.
You have the foot drop on your feet too.
Yeah, both.
So I'm going to talk to a physical therapist,
see if they can make some splint for your feet.
Can you move your hip?
A little, a little bit, like this.
Okay, the other hip too.
For this one, this one is a little bit harder
because this one hurts a little bit more, the hip.
Where do you have the pain?
Like in the joint itself.
In the hip joint?
Yeah.
So those knees, can you straighten them out?
I used to be able to.
I just lost a lot of strength in them
because I haven't done exercises with them,
so I lost strength in them.
That's why I really do need physical therapy
to get the strength back.
So what you need to do is,
you got all day to do your exercise on your leg.
Movement of the hip is going to be very important.
And let me see how much you can do with this hip.
Okay.
Move this leg.
And here you got a bunch of skin issues
that has been chronically infected.
So it just oozes all the time?
Yeah.
You need to get all this infection under control
because, you know, that is going to be a problem.
Okay?
Come in.
Careful with the dog.
Hey, Abby.
Hey, doctor.
Hey, man.
Hi.
You doing all right?
Hey, man, how you doing?
We need to work on getting him to move the hips
and his joints are freezing.
And he has a significant foot drop.
If we get some right angles made for him
to push him up a little bit.
See if we can work on those issues.
So we got a lot of territory to cover,
but, you know, you have to be the driving force and all that.
And do your own part because, you know,
we can give you the roadmap,
but if you don't do the hard work,
you're not going to benefit from it.
Yeah.
You got to work hard at it when you can.
Yeah.
Milestone will be for him to get to the point
that he can put at least feet on the ground and stand up.
Get you sitting at the edge of the bed here
within the next 30 days, maybe?
Being able to have that kind of trunk support?
Yeah.
There's a possibility to be able to get out of this bed.
Yeah.
It's going to be very difficult for you to stand up and walk.
Yeah.
So we need to cut back on whatever we give him now
to make sure whatever he eats is a protein, no carbs.
Okay?
All right, let's see if we can make some progress.
At this point, everything is up to you.
You have to work hard on your physical therapy as well.
Yeah.
Okay?
This is a very difficult situation that we have.
His condition is much worse than I thought.
Abby has lost a lot of muscle.
He didn't have any reflexes on his joint.
So he has frozen joint in the hip and both knee
and both feet are dropped.
His hygiene is very poor.
You can tell with the smell in the house.
All those bacteria growing on the side of his thigh
and hip area indicate that he probably has
serious skin issues in his back.
The starting point will be to lose some weight
and increase his mobility.
So we're going to work with him and I'm going to be optimistic
and see what kind of progress he's going to make.
But at this point, this is going to be a very difficult situation.
