How to perform a Thyroid Gland Examination - Clinical Skills Revision
The thyroid examination is one of the first sessions of the clinical skills block for medical students at Warwick Medical School - largely as it touches lightly on to other clinical areas, such as the cardiac examination, and the peripheral neurological examination making it an excellent starting point for building further knowledge
This is a clinical examination of the thyroid gland is performed by Dr James Gill following the approach in Macleod’s Clinical examination.
------------------
Please note that there is no ABSOLUTE way to perform a clinical examination. Different institutions and even clinicians will have differing degrees of variations - the aim is the effectively identify medically relevant signs.
However, during OSCE assessments. Different medical schools, nursing colleges and other health professional courses will have their own preferred approach to a clinical evaluation - you should concentrate on THEIR marks schemes for your assessments.
The examination demonstrated here is derived from Macleods Clinical Examination - a recognised standard textbook for clinical skills.
Some people may experience an ASMR effect from watching this medical clinical examination

hello my name's dr. Gill I've been asked
to an examination of your thyroid today
would that be okay so just to clarify
that's going to involve me having a look
over your hands taking your blood
pressure having a look at your eyes and
mouth tapping over your chest pushing my
hands on your neck and maybe look at
your reflexes would that be okay before
we carry on could you confirm to me your
name and date of birth
okay so to start off you put your hands
straight out in front you don't see any
problems then if you can turn your hands
over similar don't seem to be any issues
so you just touch over your hands
squeeze the tips and if you could put
your fingers together like so for me
that's once again your hands although
out in front of you and I'm just going
to put a little paper on that okay and
if you could keep your hands there I
just want to check your topsis okay
relax for me
let's do both at the same time
okay so good so I'm just going to take
this arm that's okay and go a little bit
higher up and check your pulse there as
well just seeing how they differ that's
fine
now if I could I'd like to do a blood
pressure please click each take a jump
jump off if you had your blood pressure
done before okay and do you have any
problems with either of your arms so we
don't want without the stethoscope and
then once with okay so if you just relax
that arm down so just got to put that up
first
that seems fine now we just can't do the
same again but this time with the
stethoscope listening
super okay there's no problems there at
all put that back so or dude we'll have
a look at your eyes now if that's okay
so if you could look up for me and then
down that's fine I'm just going to step
behind and have a look at your eyes from
behind as well so if you put your head
back please and I can't see any problems
that whilst I'm at the back I'd like to
put my hands on your neck to check for
any lumps and bumps would that be okay
tell me that any discomfort at all don't
shrug your shoulders up please that's
fine you could just swallow fun with
that okay all right so we'll do the same
again you could take a sip of water
please okay hold it in your mouth and
then swallow okay that's fine there's no
problems there
so we're going to do a few tests with
your eyes now if that's okay yeah so if
you keep your head still and follow my
finger okay that's good I'm now going to
say if there's any signs of double
vision so once again keeping your head
still follow my finger now if there's
any double vision at all any double
vision at all
no problems for that zipper okay so if
you could have from white for me so have
a look in the back of your mouth say I
said we've got no problems with that
lace is lie back on the bed if that's
okay um I'll have a listen to your
throat so if you could take a deep
breath in please and hold it that's fine
and then again a deep breath in and hold
it that's fine
breeze away normally dad's also fine so
to finish off this part I'm just kind of
tough over your chest if that's right so
again if you did lean back for me super
there's no problems there thank you so
just before I look at your legs further
she's gonna press on the inside of your
ankle and there's no problems there and
same again okay so if you could swing
your legs around for please okay so I'm
just going to take your hand and let you
let me take the entire weight okay one
two three perfect okay what do the same
again one two three the same again one
two three
brilliant so just take your arm like so
I'm just gonna sit good loose and floppy
there we go excellent and Sam I cannot
say no to pick up the arm
okay so relax levitate the weight superb
thank you Sam again just keeping your
legs nice and loose okay and one more
okay if you could just kneel on the bed
point please and we're going to check it
their reflexes to your ankles so I'm
just going to squeeze your calves make
sure your feet oh that's fine okay so
we're into strike over the back of you
tendon and we check the reflex once more
okay that's super so I haven't found any
worrying features in the examination do
you have any questions for me super
thank you very much your time
you
you
