Leonas, Leones, welcome to a new video, welcome and welcome one more week, one more day,
once again to this channel and we are going to give it a go to the SSM, which is the exam to enter
the specialty in Italy or what would be commonly called the Italian MIR. Many people have been
contacting me for weeks and months and especially in light of the last MIR that you have done right now,
many people have written to me about the possibility of doing the specialty in Italy, you have seen my
videos, you know that I am doing the specialty here, you want to look for a plan B or Italy has
suddenly become your plan A, so you are interested in coming to Italy, well, wait no more,
do not wait any longer because we are going to start right now a series of videos dedicated exclusively
to the medical specialty in Italy and I am not only going to be telling you curiosities about the
specialty if I am going to be explaining and teaching you completely free of charge, I just ask you
to leave me a good like and subscribe and share this video with someone who
may be interested in how to transfer your medical record from anywhere in the world to Italy
to be able to exercise as a doctor and to be able to get your desired specialty place and become
a specialist in a super nice country where you eat a lot of pizza and a lot of pasta.
Okay, as we used to do on this channel, we are going to go from the microscopic to the macroscopic,
therefore we are going to talk about a little history, we are going to understand exactly when and why
this exam began, the SSM, okay? SSM means School of Specialization in Medicine,
okay? What does it mean? School of Specialization in Medicine, it is very important that you know that
when you are doing the specialty in Italy it is a scholarship, okay? It is not a job as such,
it is a scholarship, therefore you belong to a university and that university has agreements with the
hospital, they pay you, you charge everyone the same, we will talk about this later, but you should know that
you are a university student, therefore you are going to do the specialty in a specialty school
and this is essential that you know. Okay, all this exam to enter the specialty in
Italy began in 2014, therefore not so many, many years ago and this is very interesting and
you are very interested if you have just done the SSM and right now I will tell you why. Before doing this
exam, what I saw in Italy is a little bit what exists in other Nordic countries, for example
in Germany, in Sweden, in Norway, in Switzerland and so on. You had to do merits in the service,
at first you started above all if you wanted to stay in the hospital where you were studying,
the university had agreements with this hospital and you want to do the specialty in our hospital,
what you had to do was go to work for free, do internships, do internships in the
service where you wanted to get a specialty place. So when you had been for a long time,
okay, a long time can be from four years while you were doing the degree, until after staying a
couple of years once as a doctor helping the needs of that service, above all you had the
figure of the baron, the supreme super boss of the service or the supreme super boss of the service,
who had you a little enslaved. Suddenly he told you, hey, I'm going to make you a contest, okay, the
contest, whenever you hear about a contest in Italy, in your head it has to sound like opposition,
I'm going to make you a contest, a kind of opposition, a little bit of a bath, in which I'm going to
ask you the questions so that you have a place to enter the specialty. It was a little bit the
legal, paralegal, illegal method from below the table to get your specialty place,
so let's say you were cleaning the shoes of the boss or the head of service for a long
time and then he told you one day, hey, if you want, we need a new resident of this
specialty, we are going to make you this position, we are going to do this exam for you. You did the exam,
you entered and then you could do the specialty. Therefore, it was, let's say, a favor that the
head of service did for you. But all this changed in October 2014 when the first national
exam was held, the first exam, the first position, the first Italian MIR at the national level. At
first it was a bit messy because there was a common part, then you had a specific part,
depending a little on what you wanted to do. You had either a surgical part, or a medical part,
or a service part, well, a little bit of a mess and over time it was adapting to what is
today the Italian MIR exam. At first it was not very well accepted, especially because these
head of service saw that they were losing strength over their residents because they no longer allowed
them to enter the specialty, but it was a national thing, a national level exam. Therefore,
a resident can come to you from any part of Italy or from any part of the world. So
they ran out of chanchullo and they ran out of being able to mongolize the residents so much. Although
unfortunately there are many head of service left who think that they are the bosses of the mambo and that they
can tell you whatever they want without consequences, thank goodness that there are different associations and
that they do a lot of work and are getting the residents to have the rights
that we should have had for a few years now. But hey, the situation is not
as bad as it looks. When is the MIR exam held in Italy? The contest or SSM is held
once a year and it is usually in June. And I say usually because sometimes this
exam call comes a little late . If suddenly there are irregularities or things that are not entirely clear, so
people say, hey, we want explanations for this, it may be that the exam comes late,
but normally it is done in July. The exam consists of 140 questions with 5 possible
alternatives and you have 210 minutes to take the test. It is a test that is done on the computer
in different faculties in different cities in Italy and you cannot bring neither paper nor pen
to take the exam. There are some people who have asked me about this so I
answer you now, no, you cannot bring neither pen nor paper nor anything. You can bring glasses,
the plugs, I took them once but I didn't finish using them either because they were a
little weird for me. And then you can take a bottle of water without the labels and a piece of fruit.
But hey, don't worry about the exam because we will make a video dedicated 100% to how the exam is,
how to face it, what different academies there are and why right now is a perfect and ideal time
if you have just applied for the MIR to apply in Italy to get your residence permit.
The test has a price of 100 euros and although you have to do a lot of paperwork before,
from approval, recognition, language level exam that you have to get a C1,
I already tell you, there is a lot of management, a lot of paperwork and you can get lost,
because well, I got lost a lot. In fact, when I started doing all this,
I did not find any information. Surely there would be some resident in Italy,
some Spanish resident who would have arrived before me. The truth is that I could not
contact anyone and I took out the chestnuts from the fire myself. So so that you don't have to
go through the same chaotic situation and this odyssey that I went through, don't worry
because I'm creating all these videos, okay? So that you can see them yourself and so that you can
get to Italy and become a resident as cheaply as possible. It is not free because there are
some procedures that you have to pay for, but it is the least headache possible.
Hey Pau, I've heard that the exam is much easier in Italy, that entering the specialty is
easier and well, what can you tell us about this? Well, look, considering that I have done the
MIR exam twice in Spain and I have done the MIR exam three times in Italy. In fact, I made a
quite emotional video in which I talked about entering the fifth medical specialty, okay?
Considering that I have faced this exam several times, I can tell you with
knowledge of the cause that the questions are not at all easier. In fact, the questions
have the same level of difficulty. There are many questions, in fact, that are imported from Spain.
In fact, this entire system, this entire national competition system, is an imported system from
Spain and that is also why there are many academies in Italy that are exactly the same as
in Spain and the manuals are practically the same, if not the same, only translated
into Italian. But Pau, I've heard that the exam is easier and this is exactly where it comes from.
Well, it can surely be because there are 140 questions, you have 210 minutes to do the exam,
therefore you have less time, but also fewer questions, therefore you have to be
focused for fewer hours and less time giving it to the coconut, therefore it is a much
simpler thing and also surely because in Italy you can earn your life very, very well as a
general doctor. In fact, it is very normal to finish the degree and start working for a year while
preparing for this opposition exam, taking into account that you have very good working conditions
as a general doctor and that you have many more places, this means that there is much less
competitiveness, therefore, situations as peculiar as the following occur.
For example, in the SSM contest of the year 2022, which was held on July 26,
2022, 15,873 doctors were presented and there were 14,378 places, that is, 1.10 doctors per place.
However, in Spain in the year 2022 there were 27,774 applicants for 11,171 places, that is,
there were 2.48 doctors per place. And beware, a very important thing, in the places that I have told you before,
in Spain they are also within that number, the places of family medicine, in Italy we are
talking only about hospital specialties. In Italy, family medicine has a diverse route,
it is a course that lasts three years, there are also regional oppositions, if you are
interested in talking about this I could do it perfectly and then all the places that I have
spoken to you about before are all for hospital specialties. A moment ago we were talking about the fact that in
Italy you can earn your life very well as a general doctor, well, you know that without
killing yourself at work you can easily earn about 65,000 euros gross per year. Yes, it has been
perfectly 65,000 euros gross per year. In fact, I have a friend who, working a little more,
therefore working in two places, managed to go from 100,000 euros gross per year. Obviously,
the more you earn, the more taxes you pay. But hey, in Italy there are super interesting conditions,
such as now with the new government, they have achieved that if you bill 85,000 euros or less,
you pay only 5% of 78%, well, a bit of a mess, but hey, that in short you pay very
few taxes in Italy. For this reason, there are many people who decide to work for one,
two or three years before entering the specialty. And speaking of the specialty, did you know that last year
in Italy there were more than 500 places left unoccupied? Of course, you will tell me, but hey,
surely they were places in super crappy places, or people who don't want to go, or people who are
specialty schools, as we said before, let's remember that in Italy they are specialty schools
where they don't train you so well and that's it. Well, no, look, for example, where I was before,
and I say I was because I have changed places, but hey, I will tell you this later,
during the last two years no one has come to do the specialty, and it is a place where you could
train very well as a thoracic surgeon, you did a lot of work, which is super important. Well,
last year there were places of surgery, not only thoracic, thoracic surgery, general surgery,
a lot of surgeries, emergencies, anesthesia, services, well, you had a lot of specialties that
were not occupied, a lot of free places, this is crazy, but well, well, well,
before we get in the car, we are going to go in parts, as Jack the Ripper said, we are going to finish
today's video because, well, it's already coming out a few minutes, I wanted to make it a little
shorter, but I really wanted to tell you these things, okay? I'm going to be uploading videos,
as I told you at the beginning of the video, during these weeks, this week I'm going to try to get
also how to approve and how to recognize your title to work in Italy, because it is very important
that you know that if your title comes from Europe, okay? I don't care if it's approved to work in
Spain, no, because your title comes from outside of Europe, okay? This is very important, but hey,
I'm going to be telling you exactly how to recognize and how to approve your title. I have done both
things, it is not at all crazy, and we will talk about all the processes that have to be done, where you have
to send your titles, with whom to translate it, I will advise you to translators of confidence, etc.,
etc., etc. My goal with this series of videos on how to enter the specialty in Italy is to
help you get to where I am right now, as R3 of thoracic surgery in Milan,
obviously the specialty that you want and in the city that gives you the desire. If you are very
curious to know exactly how much a resident surgeon in Italy earns, you can take a
look at this video and above all subscribe because what really comes to you will interest you a
lot. Leave me a good like if you have come this far, leave me a written Italian alpaca if
you have reached the end of the video, thank you very much, subscribe if you were not and we will
see each other with more science, with more medicine and with more things that go through my head. Until then,
may science accompany you.
